Godparent & Confirmation Sponsors (STM Parishoner/non-STM Parish)

Congratulations on being asked to be a Godparent or Confirmation Sponsor!

Many parishes ask, as a member of St. Thomas More, you provide proof that you are in good standing with the Church and meet the requirements of this role. We are happy to provide this information on your behalf on the following two conditions.

1. You are a registered member of our parish
2. You meet the canonical requirements for a Godparents/Confirmation Sponsor (a checklist of these requirements is included on the forms below)

Please print off the appropriate form, fill it out completely and bring it to the church for the pastor’s signature and Church seal. When bringing us the form, please indicate where you would like it sent once it is ready – an addressed envelope is ideal. We will process it and send it on its way as soon as possible.
